This section of your application, worth 15 points, addresses the environmental asthma management care and services that receive health care reimbursement or, if coverage is not yet available, addresses the effort underway to establish coverage to sustain environmental asthma care.

Area 3. Sustainability (15 points)

Application Guidelines Evaluation Criteria
  • Describe current and long-term funding sources, revenue streams and other arrangements that sustain your and your partners’ ability to deliver comprehensive asthma care that includes robust environmental care and services.
  • Describe specifically how you finance the environmental components of asthma care, which components of environmental care you can reimburse currently, and strategies your asthma care system is pursuing to help achieve reimbursement for the environmental components of asthma care.
  • If applicable, describe payment and delivery models (e.g., shared savings, patient-centered medical homes, accountable care organizations) currently in place to support comprehensive asthma care services in your community.
  • Describe how your asthma care system (i.e., you and your partners) track program-wide costs, savings and return on investment and how you use such data to attract funders, supporters and financial sustainability.
  • Describe your organization’s efforts to drive expanded reimbursement for in-home environmental asthma care services for people with not-well to poorly controlled asthma in your community.
  • The applicant demonstrates a sustainability strategy, including having mechanisms in place to sustain environmental education and interventions as needed.
  • The applicant and its key partners analyze return on investment from their asthma care system, including the environmental components, on a routine basis and use this information to advocate for expanded funding where the program’s efficacy supports it.
  • The applicant and its partners can demonstrate cost savings or a positive return on investment from its comprehensive asthma care efforts against a baseline measure.
