Care (COPD/Asthma Respiratory Enhancement) Program
CARE coordinator and staff provide care, service and education to appropriately identified patients with COPD/ Asthma. Patients are accepted into the CARE program with their PCPs approval. The purpose of CARE is to decrease morbidity/mortality, enhance quality of care, and improve access to COPD/Asthma medications and education.
