CARE coordinator and staff provides care, service and education to appropratley identified patients with COPD/ Asthma.  Patients are accepted into the CARE rogram with their PCPs approval.  Te purpose of CARE is to decrease morbidity/mortality, enhance quality of care, and improve access to COPD/Asthma medications and education.
