The In-Home Pediatric Asthma Program uses home visits by an Asthma educator and a community health worker to provide education and interventions to reduce triggers to improve self-management of asthma by children and families, to improve quality of life, decrease missed days of school, unplanned doctor visits, use of emergency room, and hospital visits.
