The Asthma Safe Kids Pilot Program was developed with the goal of providing in-home asthma education and environmental assessments that would help to reduce ER visits and Inpatient hospital stays. This program works in collaboration with a Medicaid funded health insurance company, nurse-managed health centers and primary care providers.
Our goal is to reach the most severe asthmatics that are enrolled in our partner's health insurance and provide them with education and follow-up services for 6 months.
