Advocate Health Care's Asthma Program aims to decrease ED re-visits and inpatient re-admissions for asthma by following an approach consistent with the NHLBI guidelines.
Advocate Health Care consists of 8 acute care hospitals, 3 large medical groups, and home health care. Program goals include: Decrease percent of Inpatient re-admissions for Asthma within 30 days of discharge, decrease percent of Emergency Department re-visits within 3 days of last ED visit, Decrease rate of Emergency Department re-visits within 60 days of last physician visit, and decrease number of ED visits for medical group patients. The program follows a consistent approach that is consistent with the NHLBI guidelines throughout all areas of care. Improvement in outcomes have been significant.
