The Asthma Improves with Management (AIM) Program provides comprehensive, pediatric asthma management education to high risk, underserved minority families in a culturally sensitive manner. Our goal is to reduce asthma disparities in the minority communities of Trenton through education. The AIM Program provides families in the Trenton area with education, resources and support as they learn to control and manage their child's asthma. The AIM program receives referrals through partnerships with community programs, Hospital Emergency Departments, schools, childcare centers, as well as individual Healthcare providers. Families can choose to attend the Community Education workshops, or to enroll in the Case Management program. The AIM program tracks outcome data by utilizing quality of life measures both at baseline and at program completion. AIM Education is provided in both English and Spanish. Based at Henry J. Austin Health Center in Trenton, NJ, AIM is available to all families in the Trenton area regardless of their Healthcare Provider affiliation. AIM is made possible through a grant from the NJ Department of Health and Senior Services, Office of Minority and Multicultural Health.
