Should I report back to clinic after the home visits have concluded? What should I include? How do you guard privacy of info?

Yes; always report back after you've made a home visit.  Share the general findings of your home visit and the results of your intervention.  Try formating your feedback with "who, what, where, when and how" as a guide.  Guard privacy by assigning patients identification codes.  In some case, you may have to physically carry data updates or reports to your partner.  Good luck!
