In search of survey tool

I am looking for a survey to determine awareness, and knowledge about asthma triggers (including climate) and ways to prevent asthma attacks in children. Is anyone uaware of any survey tools that have been used with Enivronmental Justice communities around this topic?  Please let me know.  Thanks.

ACU EPA project has developed an electronic asthma clinical decision support tool for the EHR platform. Currently it is pilot tested in 32 health centers using GE Centricity. We want to embed an asthma control screening tool. The ACT is free for children ages 4-11, but the tool has associated cost per use for ages 12 and up. We have been looking at the ATAQ, but I am unable to find copy of tool and if it is copyrighted and who to contact.  Is anyone using this tool or any other validated literacy screening tool for asthma control? All assistance valued.

Dealing with Transient Living Situations among Underserved Populations

We work with several community asthma programs that provides in-home interventions through multi-year home visits for an underserved community. Many of the programs do report that families are lost to follow-up but some programs diligently track the children. Many of the children live in transient households – for example, a child could move up to move three times in one year by living with mother, father or grandparents; as a result the program will provide three sets of home visits, trainings and supplies.

ISAAC studies have commonly used age groups of 6-7 years and 13-14 years for describing the burden, why were other age groups not taken into account ? Is there any scientific justification for that? Some studies have also used age groups of 3-7 years, 8-14 years and 15-17 years to find out the difference in prevalence using ISAAC questionnaire as study tool.
