Community Care of Wake and Johnston counties

We are a group of community nurse case managers serving Carolina Access medicaid clients. We do home visits on clients w/ asthma providing mattress covers and education as well as enviromental assessments on many by a trained enviromental specialist. We follow up x 2 years re enviromental modifications and health results.
