What tool do the CHW use to measure quality of life and how do they collect their data?

 

Thank you for the information. How is the SF 12 survey scored? Are there ranges that determine if quality of life is acceptable.  Or do you just look for changes in the scores when you give them the survey periodically?

Are the CHWs given individualized information regarding triggers for the patients?  We are recommending SPT or specific IgE blood testing in the development of pathways.  Has anyone else incorporated such a measure?

Remember that these individuals are not clinically licensed, so they do not use a formal tool to measure quality of life, like a SF-12.  We are working to get them all I-pad or other mobile unit connectivity to the care management platform- so they are directly connected.

What timing- ECA is currently moving toward a fully tablet-based energy audit of the home, including noting all reasons for rejecting a home from pure weatherization- mold, water leaks, overwhelming pest presence, etc. We use Salesforce as the host database for the weatherization data- what sort of platfrom does Caritas use for care management?
