Great question!  The Community Health Workers in the program refer participants to a wide range of supportive services including weatherization, WIC, food assistance, etc as needed.  We are also working with the RI Alliance for Health Homes for a "One Touch" direct referral program that links with a variety of home-based health, energy, and housing services.
