Thank you for the question. Initially it will probably be by phone and letter; we don't have direct access to electronic health records but I am very interested in ways of leveraging Maryland's robust Health Information Exchange at some point in the future. For now, though, it will be by mail and phone.

Q: What is the system of feedback to the pediatricians from the Community Health Workers/Nurses doing the in-home assessments?
Q: Does Maryland have multiple healthplans providing Medicaid managed care coverage to patients? If so, how are the health plans involved?
With Rhode Island's HARP
With Rhode Island's HARP model, the primary care offices receive a secure fax with a detailed report of the home visits, including what the Certified Asthma Educator and CHWs saw and any items or issues that would be helpful for primary care to know about - including scores on asthma control tests or any issues around medications. On the first visit participants are asked about their Asthma Action Plan, and if they cannot produce one they are considered as not having one and the CHW will call the primary care office to schedule an appointment to get an asthma action plan.

In MO, we have developed a standard reporting tool for sharing the results back with the health care provider. PLease contact John Kraemer PhD for details. John leads many of our home environment assessment programs across the state and trains the majority of public health professioanls to do home environment assessments. I would also recommend Kevin Kennedy at Children's Mercy in Kansas City. He certainly has examples of their report-back tools.
