How are you merging your data from different sources and complying with HIPAA restrictions?

I recently asked Jill Heins-Nesvold from the American Lung Association of Minnesota this question and she shared her experiences about merging data and complying with HIPAA restrictions.

In our program, HIPPA issues have never been a barrier to accessing data.  Depending on the situation and purpose, we have 1) included data sharing permission in parent consent forms; 2) asked for the data without personal identifiers; and/or 3) signed a vendor agreement with the health plan which has allowed us access to administrative data.

How does your program merge data from multiple sources and comply with HIPAA restrictions?

could you send me a copy of your patent consent form  used at ebailey@co.delaware.in.us

I am trying to decide what needs to be done in order to be HIPPA compliant in regards to get a doctor's physician’s signature to sign off for the program participants to confirm they have been diagnosed with asthma.
