We encourage the member to work with their primary care provider to create an asthma action plan. For the members with severe asthma, our nurses make it a goal with the member to work with their provider to develop an asthma action plan. When this goal is established, the nurses mail the primary care provider a blank asthma action plan to complete with the member during their next office visit. However, separately, the nurses plan the care based on their telephonic assessment. Additionally, the community health workers, that work with the members in the home, promote the use of their action plan. Finally, the asthma educators encourage the development of an action plan during the asthma classes.
