
Cambridge Health Alliance (CHA) is an integrated health system that includes three hospitals, more than 20 primary care practices, and a statewide Medicaid-managed health program (Network Health). The objective of our asthma program is to perfect asthma care for patients ages 3-18 through the use of an electronic patient registry, redesigned process, and new approaches to partnering with patients. In addition to the registry and other systems changes, program activities include patient education, provider training, and the Healthy Homes home-based environmental intervention. Evaluations based on ~1200 patients and their families have shown a positive impact on hospital utilization, ED and urgent care visits, and office visits.
