I'm interested in home-based/community asthma programs that have partnered with health plans (govt or commercial insurers), employers or other payers for payment of services (fee-for-service or other arrangements), referrals, data sharing, etc. Examples, success stories, lessons learned...?

Through the Affordable Care Act, each state will be organizing a provider Exchange.  Finding those providers will be a good way to go moving forward beginning this fall.

You could look at Sinai's Asthma Care Program as an example of how we have been able to work with Medicaid to fund a CHW asthma program. www.SUHIchicago.org
