For each presenter:

1. Does your asthma action plan have a specific section for applicable triggers?

2. What % of students with asthma have an AAP on file with their school?

1.MCHD uses a Nursing and Environmental Action Plan to work with the families.  We do identify applicable triggers and how to mitigate as part of the action plans.  The asthma action plans are developed by the provider and our staff give input to the provider to include in action plan.   Our staff also provide a summary at the close of the visit. 

2.  All students in our schools are supposed to have AAP on file, but I don't know how close to 100% is it.  I will ask our nurses for their sense.

no, we write them in, as we review w/ patient/family

2. What % of students with asthma have an AAP on file with their school?

<5% due to lack of electronic communication to provide updated AAPs.

3. Does the AAP also serve as the formal medication authorization?

AAP serves as the formal education tool for home visits.  Pediatrician signs before CHW uses in home

More information from our nurses 

1. Our AAP does not have a section on specific triggers.  We call the AAP a "medication guide" and we try to keep it very simple. In some cases, with families who do not read, it is pictorial.  We teach in depth on triggers/irritants/allergens and have a section of the notebook dedicated to that.  We also have a Family Action Plan that outlines their goals to reduce triggers/irritant/allergens in the home.
2. We make sure that all of our clients have their Rescue Medicine and the Rescue Medicine script at their school.  Some schools require an Asthma Action Plan and some do not.
3.  Our AAP (or "Medication Guide") is based on the rx from the provider but does not serve as the formal medication authorization
