Through our different studies, we continue to look at health outcomes, including healthcare utilization, inhaled steroid use, and missed school days.  For clinic metrics, we are primarily focused on tracking referrals and visits, and periodically look at a sample of patients to track outcomes.

We also use citywide data to show trends, and to make the case about expanding different components of our program.

Messages that currently seem to resonate include: improving population health, decreasing healthcare costs, transitioning to place-based primary care, care coordination, and school attendence.
