The vision of the Asthma Program of Union Hospital is to collaborate with patients, families, physicians, and health care proffessionals to bring educational excellence in controlling asthma. Our goals include working one on one with children and adults affected by asthma utilizing an asthma action plan. Our second goal is to educate persons that interact with individuals with asthma including- physicians, nurses, schools, daycares, parents, friends, and family memebers. Services offered includes bi-weekly visits with a certified asthma educator, conference with primary care physicians to develope individualized asthma action plans, school and daycare inservice, and community outreach. Enrollment in the program can be made through referrals by providers, nursing, and allied health professionals.
