The Child Asthma Link Line is a telephone-based care coordination system that connects families affected by asthma to community-based educational, medical, and social service resources.

The Child Asthma Link Line (Link Line) is a telephone-based care coordination system that connects families with asthma to community-based educational, medical, and social service resources. Staffed by three Asthma Care Coordinators trained in asthma management, the Link Line gives families the tools to improve their child’s asthma care by improving the connectivity of Philadelphia's diverse asthma programs.

The Asthma Link Line is a service sponsored by the Health Promotion Council, a nonprofit organization whose mission is to promote health and prevent and manage chronic diseases among vulnerable populations in Southeastern Pennsylvania.
