The Monroe Plan Improving Asthma Care for Children program targets high-risk children enrolled in Medicaid and State Children's Health Insurance Programs under managed care to improve the health and quality of life for children with asthma, develop and sustain partnerships with key community stakeholders, and institutionalize innovative clinical models in asthma care.
The Monroe Plan Improving Asthma Care for Children program was designed and implemented using a creative approach to addressing improved asthma care management. The Rochester, New York-based plans targeted high-risk children enrolled in Medicaid and State Children's Health Insurance Programs (SCHIP) under managed care. The initiative seeks to improve the health and health-related quality of life for children with asthma, to develop and sustain partnerships with key community stakeholders, and to institutionalize innovative clinical models in asthma care.
