Promotes continuum of health provider access with asthma advocate on-site while promoting clear communication with licensed clinicians. Conduct needs assessments by identifying medication adherence structure and barriers with medical literacy during doctor's visits. Educating parents on policies and practices that are available within compliance needs of asthmatics.
Providing basic asthma education on a regular basis to our clinic's patients with the goal of reducing emergency room visits, unscheduled office visits, and the need for oral and inhaled corticosteroids.
The program aims to improve health outcomes of Hispanic and African American patients who struggle with asthma. Program combines culturally appropriate activities, such as age-appropriate member educational materials, nurse outreach by telephone, optional home assessments and physician engagement.
The Asthma Disease Management Program operates within the Quality Improvement and Care Coordination Departments. The goal of the Asthma Disease Management Program is to improve the health status for Meridian members with asthma using a multi-disciplinary, population-based approach and to manage health care costs by promoting evidence-based diagnosis and treatment while assisting members to achieve optimal control of their disease through adherence to treatment plans including self monitoring and appropriate medical testing.
