South Bronx Rising Together is a collaborative network of program providers and community stakeholders. This group will work together to create a community that is college and career ready by leveraging the expertise of a network of families, educators, business leaders, community advocates and service providers to support the lifelong success of families and youth in the community. We have five Collaborative Action Networks that meet monthly to work on projects specific to each goal. Goal #1- All are Healthy is focused on reducing the rates of asthma related ER visits and hospitalizations in children who live in Community District 3 in the South Bronx.

The East Harlem Asthma Center of Excellence (EHACE) provides individuals and families with FREE services to help control asthma and improve their overall health. These services include one-on-one asthma counseling, community health workshops and referrals for indoor pest and mold removal. EHACE is a program of the New York City Department of Health and Mental Hygiene’s East and Central Harlem District Public Health Office.

The Merck Childhood Asthma Network (MCAN) partnered with The George Washington University (GW) School of Public Health and Health Services, the RCHN Community Health Foundation and Rho to implement the Community Healthcare for Asthma Management and Prevention of Symptoms (CHAMPS) project in three non-profit, Federally Qualified Health Centers (FQHC).This initiative in community health centers (CHC) is designed to demonstrate how tailored, evidence-based asthma management programs proven efficacious in controlled trials, can be implemented in real-world settings where many children and families most in need get care.
