St. Louis County Community Based Asthma Care

The Healthy Homes Program provides education materials to caregivers and persons with asthma on the management of indoor asthma triggers. We provide in-home consultations and in-home assessments as an element of the education process. In addition, classroom training is provided to healthcare professionals, including school district staff, on the management of indoor asthma triggers.

Michigan Department of Community Health Asthma Prevention & Control Program

The Michigan Department of Community Health’s (MDCH) Asthma Prevention and Control Program (APCP) is a comprehensive program responsible for addressing asthma throughout the state of Michigan. This is accomplished by providing leadership, coordination of efforts, asthma expertise, and resources. The program focuses on systems change, integration and replication of successful programs, garnering policy improvement through strategic partnerships, and using surveillance data to expand program reach and address disparities.

County of San Bernardino Department of Public Health, Coordinated Asthma Referral and Education (CARE) Program

The Coordinated Asthma Referral and Education (CARE) Program provides free home assessment, health education, athma management and supplies for children under 18 who have asthma in certain geographical locations in San Bernardino County. Please call 1-800-782-4264 for more information, Monday-Friday from 8-5 pm.
