Michigan Department of Community Health Asthma Prevention & Control Program

The Michigan Department of Community Health’s (MDCH) Asthma Prevention and Control Program (APCP) is a comprehensive program responsible for addressing asthma throughout the state of Michigan. This is accomplished by providing leadership, coordination of efforts, asthma expertise, and resources. The program focuses on systems change, integration and replication of successful programs, garnering policy improvement through strategic partnerships, and using surveillance data to expand program reach and address disparities.

County of San Bernardino Department of Public Health, Coordinated Asthma Referral and Education (CARE) Program

The Coordinated Asthma Referral and Education (CARE) Program provides free home assessment, health education, athma management and supplies for children under 18 who have asthma in certain geographical locations in San Bernardino County. Utah Asthma Program

The Utah Asthma Program is located at the Utah Department of Health in the Bureau of Health Promotion. It began in December 2001 with the intent to develop state capacity to address asthma, including an occupational component. The program is funded through the Centers for Disease Control and Prevention (CDC).

Incorporated in 1965, the agency came into being as part of a nationwide, federally-funded, community action movement whose focus was to intimately involve poor people in developing their own solutions to persistent poverty.

The NYS Asthma Program is implemented through an internal cross agency infrastructure focusing on surveillance, health care delivery and quality, community based initiatives and environmental and occupational health; and through the Asthma Partnership of New York, a public/private collaboration with the NYSDOH, New York City Department of Health and Mental Hygiene, the CDC and key statewide and national stakeholders
