Does anyone have an evidenced-based training module for an FQHC patient population for ASTHMA GROUP VISITS.

Attend a free one-day training event hosted by the School District of Philadelphia and cosponsored by the U. S. Environmental Protection Agency and the American Lung Association of the Mid-Atlantic. The training will focus on how school district personnel can take simple, low-cost steps to improve indoor air quality (IAQ) in schools while saving money, improving health, and decreasing student and staff absenteeism.

Retainment in HHP, supplies, and final survey
What is the completion rate for your enrolled families in the healthy homes asthma services that you offer?
Do you provide incentives to caregivers to finish the program?
Are the asthma trigger control supplies (ie. vacuum cleaner, mattress and pillow encasings, etc) provided to the family covered in the visit reimbursement as a recognized cost by the state medicaid program, or is this a separate reimbursement?
Respiratory Therapists/Asthma Educators
Do you have Respiratory Therapists involved in this program? Do you partner with the schools to increase awareness of the program? Looking to see if the schools can get reimbursed for the asthma prevention like we do for other medication administration/treatments through our state's Eshars.
EPSDT and CHW reimbursement
I heard the EPSDT referrred to as if it could somehow be connected to the CHW reimbursement if we developed a SPA. Please clarify, could those same codes be used?
Engaging Community Health Centers
Now that some states have more options for reimbursements, what would you suggest as the best starting places, guidance, and pitches to use with individual community health centers to get them to embrace the expanded services?

In order to realize the cost savings as calculated how did you find the high cost super utilizers for intervention. Were they referred by the insurer, hospital, provider?
