Funding
Who/what funds the Community Health Workers (CHWs)?
Who/what funds the Community Health Workers (CHWs)?
How much funding did you receive from HUD for this program? Were there any other funding sources (e.g. in kind from Siani or CHA, other local funders, etc)?
I have two questions for you:
1.) I was surprised to see the high health care utilization rates at baseline for the people enrolled in the intervention. Did I understand correctly that 68% of people who participated in this program had visited the ER for an asthma related event in the 6 months prior to starting the program?Do you think this rate was unusually high? Or typical of kids/adults living in public housing units with asthma?
What was your curriculem for the 70 hour asthma community health worker training program?
I will be presenting a session on CHW and asthma at this conference in June. Would love to get a copy of one of your photos of your CHWs with the diplomas!

Look at places like MA and RI and WA states where managed cares organizations will help pay for CHW because of the great outcomes.
LOVE the concept that PH residents who become CHW may enter into the health workforce and develop more skills!

I'm interested in home-based/community asthma programs that have partnered with health plans (govt or commercial insurers), employers or other payers for payment of services (fee-for-service or other arrangements), referrals, data sharing, etc. Examples, success stories, lessons learned...?
I’m Dr. Floyd Malveaux with the Merck Childhood Asthma Network (MCAN). We are focused on enhancing access to quality asthma care and management for children in the U.S. As a nation, we cannot continue to approach childhood asthma the same old way and expect to reverse the fact that one in every 11 children suffers from it and the asthma disparities that exist in many communities.
