The In-home Pediatric Asthma Program uses home visits by an Asthma educator and a community health worker to provide education and interventions to reduce triggers to improve self-management of asthma by children and families, to improve quality of life, decrease missed days of school, unplanned doctor visits, use of emergency room, and hospital visits.

The Bronx RESPIRAR Coalition (BRAC) is a novel 5-year clinical service program that involves systems improvement, enhanced patient centered support in the community, and clinical decision support for providers proposed by the Lincoln asthma providers’ team that has received funding by the NYSDOH asthma coalition program. RESPIRAR, which means Breathe in Spanish, is also the acronym for Resources, Education, Self Management, Prevention, and Integration to Reduce Asthma Rates.
