Community Care of Wake and Johnston Counties is a group of community nurse case managers serving Carolina Access medicaid clients that conduct home visits and environmental assessments to patients with asthma.

We are a group of community nurse case managers serving Carolina Access medicaid clients. We do home visits on clients w/ asthma providing mattress covers and education as well as enviromental assessments on many by a trained enviromental specialist.
