This enabled through partnerships with local businesses and organizations. The voluntary program enrolls adults and children who have been diagnosed with one or more chronic disease who require outpatient care. The program targets the following groups of vulnerable patients: individuals with uncontrolled or poorly managed conditions; high-risk or complex patients; patients with multiple chronic conditions; patients who are non-compliant with medications or medical appointments; patients encountering challenging social and stated barriers to care; patients experiencing frequent inpatient hospitalization or ER visits; and individuals in need of care coordination.
