Pediatric Asthma Care Management Improvement Initiative (ACMII)

Private practice pediatricians in NW Oregon and SW Washington unite through the Children's Health Foundation to measure actionable asthma care goals via a pediatric asthma registry and utilize rapid-cycle quality improvement to implement provider-led solutions resulting in high quality asthma care management that enhances pediatric asthma care delivery and improves the health of children with asthma.

Healthy Homes & Lead Poisoning Prevention Program

The program works on the Healthy Homes initiative to raise awareness of environmental health risks in homes such as lead-based paint, mold, carbon monoxide, pesticides, and hazardous household products through one-on-one family education, realtor and landlord education. To reduce or eliminate environmental hazards in the home, the program refers families to local health and housing programs to fix their hazards.

Chester Home Asthma Prevention Program (CHAPP)

The Chester Home Asthma Prevention Program (CHAPP) was a successful home-based asthma education and environmental remediation program for Chester City, PA, an Environmental Justice community funded by a three-year EPA grant administered by the Pennsylvania Department of Environmental Protection. CHAPP addressed both indoor and outdoor asthma triggers and was a collaboration between the Crozer-Chester Medical Center and the Chester Environmental Partnership (CEP).
