The primary goal of the Asthma & Pulmonary Education at New Hanover Health Network (NHHN) is to provide comprehensive care for patients with asthma and pulmonary-related disorders, including diagnostic evaluation, self-management planning and education, and monitoring.

The Sonoma County Asthma Coalition's mission is to work with key stakeholders to improve the quality of life of people affected by asthma and to advocate for policies to minimize the impact of asthma in Sonoma County.

Community Care Partners of Greater Mecklenburg County is a Medicaid Disease Management program that helps patients navigate the medical system and conducts community education programs.

The goals of the Asthma Control and Prevention program include increasing community awareness using public education; conducting asthma surveillance; developing policies which reduce the prevalence, severity, and limitations of asthma; and promoting and providing education for physicians and other health care professionals to diagnose and treat asthma more effectively.

Through both clinical care and community partnerships, Boston Medical Center (BMC) provides a full spectrum of accessible services to people with asthma within the Boston area, with particular emphasis on providing care for those who can not afford it.

Through a comprehensive evaluation and management plan, the Children's Hospital Asthma and Allergy Center helps children with asthma to avoid urgent care visits and hospitalizations.

Breath by Breath provides testing and education to students and parents at schools and patients at clinics through a traveling ambulance.The programs goal is to provide testing and education to patients who would normally not travel to the hospital to receive care. Clinics are visited mo
