The Virginia Asthma Action Plan is a document orginally developed as the Southeastern Virginia Asthma Action Plan by the Consortium for Infant and Child Health (CINCH). The plan is updated yearly and is intended for use for school age children. Schools and physician practices can print and make available the form. Copies in triplicate are available to schools and providers in southeastern Virgi

Copies in triplicate are available to schools and providers in southeastern Virginia. The form is available for free via download from the Virginia Department of Education website. It is updated yearly by the Virginia Asthma Coalition. The form is intended to improve asthma management at school, home, and in the community (sports, afterschool, etc) for children with asthma by providing a comprehensive plan and encouraging the use of appropriate medications, as well as having rescue medications available at school and community settings.
