Knox County Community Health Coalition provides training and education to community members and partners with health care providers, health plans, community organizations, and others to improve the number of people in Knox County who are actively self-managing their asthma.

Our ongoing work will include training more of our school staff to detect the signs/symptoms of asthma; increasing the percentage of individual Health Care Plans (HCPs) for students diagnosed with asthma; decreasing the number of emergency room visits and hospitalizations due to asthma by partnering with diabetes educators and PCPs; partnering with area employers to do asthma education via worksite wellness programs, and increasing the number of people who actively self-manage their asthma.

Breathe Easy San Francisco plans to combine clinical and social care services to maximize the effectiveness of the team-based approach to asthma management.

We are currently in the planning stages of development. Listed below are the program components we have identified. We have secured funding and clinic space. We are working on credentialing our staff.

Program Components

1. Design auxiliary clinical services aimed at managing asthma using chronic care strategies.
2. Develop and implement strategies to care for patients according to level of risk.
3. Combine clinical and social care services to maximize the effectiveness of the team-based approach to asthma management.
4. Develop and mentor a network of asthma advocates to serve as liaisons between the community.
