The Asthma Management Program at Children's Medical Center Dallas is a six-month, comprehensive program that focuses on asthma education, self-management skills and coordination of care for children ages 0-17 with a diagnosis of asthma.

The Asthma Management Program at Children's Medical Center Dallas is a six month comprehensive program that focuses on asthma education, self-management skills and coordination of care for children ages 0-17 with a diagnosis of asthma. Our program includes bi-weekly telephone contact with an RN AE-C certified care-coordinator to assist families in self-management strategies and review progress of environmental control plans and medical follow up. It also provides two home visits by pediatric RN''s who equips the family with education about asthma medication and triggers. The family is also provided with a peak flow meter, a spacer and asthma education workbooks. An asthma education class is offered to the families to provide additional education.
Current statistics show a 74% reduction in missed school days, an 86% reduction in asthma related ER visits, an 85% reduction in asthma related inpatient admissions, and a 15% increase in asthma Quality of Life scores.

The program has achieved JCAHO certification in disease specific care for the past six years.

Miami County Asthma Education Program is a referral-based hospital program offered free to the community that provides education to patients and their families in order to decrease ER visits and hospitalizations as a result of asthma.

Hospital based program offered free the community Physician referrls unattached ED patients. Goal to decrease ER, physician visits and Hospitalizations through education. Three one on one meetings at 2 week entervals we supply peak flow meters, spacers and evaluate current therapy and aliegn treatment with the NAEPP guidelines. Screening questionaire at first, last visit and by phone at 3 month, 6 month and 1 year for outcomes. School education done mainly for teachers and coaches.

The Childhood Asthma Initiative provides enhanced asthma care to homeless children living in shelters in NYC, including asthma education and training.

The Childhood Asthma Initiative provides enhanced asthma care to homeless children living in 30% of the shelters in NYC. The program offers asthma education and training for patients, shelter staff and clinical providers. The goal of the program in to ensure these patients are provided with current asthma management regimens to control their asthma and improve their quality of life. These strategies are based on the 2007 NHLBI guidelines. The program has been successful in classification of asthma severity and providing persistent asthmatics with controller medication regimens. The program works in partnership with other organizations to provide the general community with asthma education.
