A new program in McLeod County, this program consists of nurse home visits that will help reduce environmental triggers in homes of children with asthma.

The Boston Public Health Commission (BPHC) is the health department for the city of Boston. The BPHC started the asthma program in 1998. Program efforts are designed to: increase access to quality medical care, ensure access to community based resources, track and monitor asthma trends, eliminate racial and ethnic health disparities in asthma, increase public awareness of asthma, and promote policies to support the prevention and control of asthma.

Community Care Partners of Greater Mecklenburg County is a Medicaid Disease Management program that helps patients navigate the medical system and conducts community education programs.

Contra Costa Health Services Public Health Asthma Program provides asthma care and assistance in the home, the clinic and the schools of children within Contra Costa County in order to decrease hospitalizations, ED use and improve quality of life for children with asthma and their families.

Through a comprehensive evaluation and management plan, the Children's Hospital Asthma and Allergy Center helps children with asthma to avoid urgent care visits and hospitalizations.
