The Children’s Healthcare Access Program, CHAP is a proven medical home improvement model that helps transform pediatric primary care practices (PCPs) to become more accessible, continuous, comprehensive, family-centered, coordinated, compassionate, and culturally effective.

Chester Home Asthma Prevention Program (CHAPP)

The Chester Home Asthma Prevention Program (CHAPP) was a successful home-based asthma education and environmental remediation program for Chester City, PA, an Environmental Justice community funded by a three-year EPA grant administered by the Pennsylvania Department of Environmental Protection. CHAPP addressed both indoor and outdoor asthma triggers and was a collaboration between the Crozer-Chester Medical Center and the Chester Environmental Partnership (CEP).

Puerto Rico Coalition for Asthma & Other Chronic Respiratory Conditions

The Puerto Rico Coalition for Asthma and other chronic respiratory conditions is a non-profit organization founded in 1991 to educate, empowered and protect patients with respiratory conditions. It has worked with other groups to create a law to protect the right for students with asthma to use their medications in school, by developing a protocol. Also, it has worked with the PR Lung Association in patient education matters and activities. The Coalition delivers CME programs for health professionals as well as education to community-based groups in areas of low income and disparities for kids.
