State Plan Amendment submitted by Multnomah County Environmental Health

This is the State Plan Amendment (SPA) used by Multnomah County Environmental Health to add their Targeted Case Management- Healthy Homes Program to Oregon's State Plan under Title XIX of the Social Security Act - Medical Assistance Program.

This is the State Plan Amendment (SPA) used by Multnomah County Environmental Health to add their Targeted Case Management- Healthy Homes Program to Oregon's State Plan under Title XIX of the Social Security Act - Medical Assistance Program.

As part of the process involved in the submission of this SPA, Oregon state submitted a waiver to CMS to waive certain Federal requirements, including the ability to get reimbursement for community health workers.

This SPA was referenced in the February 13, 2014, webinar, "Effective Strategies for Obtaining Reimbursement."
