The mission of the Washtenaw Asthma Coalition is to prevent the development of asthma and to improve the quality of life of asthmatics through community-wide collaboration.
The Wayne Children’s Healthcare Access Program (WCHAP) is an independent, physician led, public-private community health collaborative. The Children’s Healthcare Access Program, CHAP is a proven medical home improvement model that helps transform pediatric primary care practices (PCPs) to become more accessible, continuous, comprehensive, family-centered, coordinated, compassionate, and culturally effective.
The Wealth from Health® program, utilizes nurses and non-clinical navigators to support, advocate for, and motivate chronically ill patients using an innovative points-driven financial rewards system. This enabled through partnerships with local businesses and organizations. The voluntary program enrolls adults and children who have been diagnosed with one or more chronic disease who require outpatient care. The program targets the following groups of vulnerable patients: individuals with uncontrolled or poorly managed conditions; high-risk or complex patients; patients with multiple chronic conditions; patients who are non-compliant with medications or medical appointments; patients encountering challenging social and stated barriers to care; patients experiencing frequent inpatient hospitalization or ER visits; and individuals in need of care coordination.
