PFS models seem to be the new big thing. Do either of you have experience with other funding mechanisms that seem to work well for asthma care?
Do you have any recommendations for cheap/cost effective databases for tracking or compiling data?
What was the reasoning for using CHWs in the Tennessee program? Did you encounter any resistance to using CHWs instead of nurses and how did you overcome that? Are there any challenges/drawbacks to using CHWs and how do you mitigate for that? What are best practices in recruiting- and retaining- CHWs?
In thinking through program staff costs - are the metrics for the cost analysis compiled by a health actuary? At what cost?
What was the intervention that was costed out for the feasibility study -- the community health worker and CHAMP program -- or did it also include any costs of healthy housing repairs?
What type of housing are they seeing patients in - private homes, rental housing, apartments, public housing (governmne financed?). What type of home environmental services are they providing beyond the assessment? Removing carpet, remediating mold, mattress covers, air cleaners etc.?
Has HUD been included in any of the housing resource/building changes as a part of the GHHI and Le Bonheur partnership?
